The Role
Guide Dogs Victoria is seeking a passionate and experienced Support Coordinator to join our dedicated team in Kew, VIC. As a Support Coordinator, you will play a vital role in empowering and supporting individuals with disabilities to live life without limits. This is a full-time position that offers a competitive salary and excellent benefits.
What you'll be doing
Collaborate with clients to develop personalised support plans that address their unique needs and goals.
Work with the client to implement funded formal supports, informal, mainstream and community supports.
Monitor client progress and adjust support plans as needed to ensure ongoing satisfaction and positive outcomes.
Coordinate referrals to service providers, including Guide Dogs Victoria, as requested by the client.
Engage in regular communication with clients, their families, and other stakeholders to ensure seamless service delivery.
Skills and Experiences:
What we're looking for
Certificate IV in Disability or Community Services or a relevant degree in Social Sciences.
Experience in a similar support coordination or case management role.
Solid understanding of the principles of the National Disability Insurance Scheme and how they apply in practice.
Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse clients.
Proficiency in using client management software and other relevant technology.
Committed to person centred service practices.
