Supportive Services Provider-Minimal Hours
Member in Wausau seeks to find a Supportive Services Provider to help her with tasks in her apartment and errands
Do you have experience working with the visually impaired?
Do want to assist a member of the community to maintain an independent lifestyle?
Do a job that makes a DIFFERENCE in someone’s life by working just a few hours a week
**This is a great opportunity for a student or a retired person looking for just a few hours per week-Flexible shift times**
Tasks:
- Cleaning/household task assistance
- Meal preparation/planning
- Assistance with grocery shopping/errands
- Transport to and from appointments and attend as needed (mileage reimbursement)
- Provide companionship and support to member
- Assistance with setting up weekly medications with member
- Medication administration and blood sugar checks
Schedule: Shift times can be flexible-up to 6 hours per week
The member would like to find a person who is reliable, honest, and has a positive attitude
**Experience with working with the visually impaired individuals is a plus**
Wage: $16.75 per hour (mileage reimbursement when applicable)
A background check is required before employment can begin

Who is Community Horizons
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.

Pay: $16.75 per hour
